We accept all major insurance carriers on this page and PPO’s. We are in the Delta Dental Network. Our office does not participate in HMO’s or Medicaid.

When you call:
Please have ready your insurance card so we can obtain the necessary information. The policyholder’s social security number and date of birth are needed to verify coverage. We will call your insurance company to determine the benefits that are available. Once we find out, we will call you and advise what they are, so you will be informed before your appointment of your particular coverage and if any deductibles and / or copays apply.
After your initial exam:
In many cases we submit a pretreatment estimate (authorization) prior to commencing treatment in order to find out what portion your insurance will pay and what your responsibility is. The insurance company mails a printed form explaining this in 3 – 4 weeks.
After each visit:
Our office will submit your claim electronically to the insurance company. We accept assignment of benefits which means we will receive payment directly from the insurance company. Be ready to pay that day any deductible and / or copay that may apply. You will be explained before the appointment if any payments are due. Please be aware that the non – payment of a claim by your insurance company makes you responsible for that payment.
